The following information is required to be submitted by all students to provide effective delivery of care and to ensure a safe campus environment for all. Students must log on to the Student Health Portal using their Monarch account username (King's College email) and password for detailed instructions.

Immunization History - Pennsylvania Law requires that all students living in campus housing receive the meningococcal vaccine. King's College requires a vaccination after the age of 16 or a signed waiver before occupancy is permitted.
Insurance information
Meningitis Requirements
The Commonwealth of Pennsylvania mandates that students living in college-owned and operated residence halls must either provide proof of meningitis immunization or sign a waiver to indicate they have been informed about the disease and the vaccine and have chosen not to be immunized.
Students who fail to submit proof of meningitis vaccination or a signed waiver may not receive a room key or be able to move into their residence hall. Students may also be placed on administrativeholdand remain onholduntil compliance is documented with the student health center.
Health insurance coverage is mandatory for full-time resident students, athletes, and international students and strongly recommended for all other students.
Proof of health insurance (a clear image of the front and back of your health insurance card) should be uploaded on the Student Health Portal.
Students Without Insurance
King's College refers domestic students and athletes to various agents for insurance options and purchases that are individually structured for students who have no other form of health insurance or need additional local or sport insurance coverage. Please be sure to fully disclose your needs to the agent when investigating purchase options.
King's College provides accident insurance for all students. This insurance coverage is an excess policy. A claim will be filed for an accident occurring on campus; however, a student's health insurance is always primary.
In the event of injury, you should contact or report to the Student Health Center as soon as possible to complete the Accident Claim Form.
